Insurance Information

We are in network with the following plans:

• Blue Cross Blue Shield of Texas:
  • Blue Choice PPO (Inclusive of Blue Precisions / Blue Options)
  • Blue Essentials HMO
  • BCBS Health Select
  • Blue High Performance Provider Network
  • Blue Premier
  • Please note that we are NOT in network with Magellan. If your BCBS Behavioral Health benefits are managed through Magellan we will be out of network for your plan.
• Medicare (traditional Medicare)

All other patients will be seen “out of network” for our self-pay rates (see fees here). We do provide Superbills to out of network patients who wish to seek reimbursement for fees from their insurance companies.

Please be aware that insurance panels/plans each have their own fees for various codes/visit types. These fees are set by the insurance companies, and are different from my out of network / private pay rates. The amount you will pay if using your insurance plan depends on your individual diagnoses, complexity of visits, and your insurance plan specifics (deductible, copay, coinsurance, etc).

Prior to scheduling, we will verify your insurance, and confirm whether or not you are wanting to use your insurance benefits for appointments.
