Please read below for important information on this page prior to filling out an appointment request form. Please note that you can fill out two appointment requests (for medication management AND therapy) if you are interested in both.

Insurance/Payment:
Pathway Psychiatry is currently in network with the following insurances:

• BCBS of Texas: Blue Choice PPO, Blue Essentials HMO, Health Select, Blue High Performance Provider Network, Blue Premier
• Traditional Medicare

All other patients will pay fees out of pocket, as outlined at this link.
